Employee Information
Open Enrollment for 2008 is Now Closed
All employees and retirees will receive a 2008 Benefits Confirmation Statement whether or not you made changes during the Open Enrollment period. If you do not receive a Confirmation Statement by Tuesday, December 18, 2007, call the Benefits Resource Line at 1-866-325-2345 and speak to a Benefits Specialist.

You have ten business days from the date of the confirmation statement to make any corrections to the elections you have made.  If any of the information is incomplete or incorrect, please call the Benefits Resource Line toll-free at 1-866-325-2345 and follow the instructions to speak with a Benefits Specialist.  Benefits Specialists are available Monday through Friday, 7:30 a.m. to 5:30 p.m. Pacific Time, except on holidays.

Now that the Open Enrollment has closed, you will not be able to make changes to your before-tax coverage unless you have a qualified life event (e.g. marriage, birth of a child, adoption or divorce).  If you have a qualified life event before the end of 2008, please contact the Benefits Resource Line at 1-866-325-2345 and speak with a Benefits Specialist.  If you add a newly eligible dependent outside of the Open Enrollment period, pre-existing conditions will apply in the PPO plans.  Other plan or enrollment changes cannot be made until the next Open Enrollment.

IMPORTANT DATES  
December 18, 2007 All employees will receive a 2008 Benefits Confirmation Statement shortly after making their 2008 elections. If you did not make any changes during the Open Enrollment period, you will also be mailed a 2008 Confirmation Statement to your home address in the beginning of December. If you do not receive a Confirmation Statement by December 18, 2007, call the Benefits Resource Line at 1-866-325-2345 and speak to a Benefits Specialist.
January 1, 2008

All 2008 Open Enrollment changes become effective. If you changed health plans, you may begin using your new plan on this date. New dependents may also begin using plan on this date.

New I.D. cards should be received by this date. If you changed health plans or added dependents and do not receive I.D. cards by this date, please contact the Plan’s Member Services Department directly.

January 11, 2008 2008 health plan premiums for full-time and part-time will be reflected in subscriber’s paychecks.
